Building the Foundation for PCMH Recognition Across Five Practice Sites
The Challenge
A multisite primary care practice sought to enroll five locations in the NCQA Patient-Centered Medical Home recognition program. However, the organization needed a coordinated approach to assess readiness, standardize workflows, organize supporting documentation, and align clinical and administrative teams with NCQA requirements.
Differences in processes across the five sites created additional challenges related to documentation, staff responsibilities, quality reporting, care coordination, and consistent implementation.
The Approach
CMAG leadership developed and coordinated a structured PCMH readiness and enrollment strategy across all five locations.
The initiative included:
Conducting readiness assessments to identify gaps against NCQA PCMH requirements
Developing a centralized implementation plan with clear milestones, responsibilities, and deadlines
Standardizing clinical and administrative workflows across participating sites
Organizing policies, reports, screenshots, and other supporting evidence required for submission
Working with practice leadership, physicians, nursing, operations, information technology, and front-office teams
Providing staff education on PCMH standards, documentation expectations, and ongoing responsibilities
Establishing routine progress meetings to address barriers and maintain accountability
Coordinating the enrollment and submission process for each location
The Results
Within six months, all five primary care sites achieved NCQA PCMH recognition. The initiative established a standardized quality infrastructure across the organization, strengthened coordination between clinical and administrative teams, and created a sustainable framework for maintaining PCMH requirements.
